Twelve Axes · One Composite
The Clarity Assessment
At the center of the program is a single, careful question: is your loved one still themselves? The Clarity Assessment answers it the only way we know how — gently, and in twelve dimensions at once.
The assessment measures cognitive functioning across twelve axes. No axis decides anything on its own. Together, weighted against a person’s own baseline, they produce a composite score — and, more importantly, a picture of a whole person changing slowly over time.
Short-Term Memory
Holding new information in mind — a phone number, a sequence, a name just heard.
Long-Term Recall
Retrieving what was learned minutes, days, or decades ago.
Spatial Reasoning
Understanding shape, rotation, and how objects relate in space.
Verbal Fluency
Finding words quickly and freely — the axis families notice first.
Facial Recognition
Reading faces and the emotions written on them.
Temporal Orientation
Knowing the day, the season, the year — locating oneself in time.
Executive Function
Planning, sequencing, and carrying a task through to its end.
Emotional Regulation
Steadiness of mood and response across the ordinary day.
Procedural Memory
The hands’ own memory — the recipes and routines of a lifetime.
Abstract Reasoning
Seeing patterns, analogies, and the idea beneath the surface.
Social Cognition
Reading a room, a relationship, an unspoken cue.
Self-Awareness
Recognizing one’s own changes — a positive indicator of self-monitoring.
Above threshold, below threshold
Any single axis below fifty triggers a formal review. Three or more axes below fifty, and a participant is below threshold — the point at which the ninety-day Grace Period begins. Read more about thresholds ›
Self-Administered Practice Screening
Experience the assessment for yourself
This practice screening mirrors the real experience — the number sequences, the words, the faces, the questions — and returns a full clarity report scored across all twelve axes. It is not the official assessment, and nothing you enter is stored or sent anywhere. It takes about six minutes.
Before you begin
You’ll move through a series of short tasks. Some are timed. Find a quiet minute, and answer as naturally as you can — there is nothing to prepare for and no way to fail a practice.
